So, what is a Multidisciplinary Team?
A multidisciplinary team (MDT) is a diverse group of healthcare professionals. MDT members combine their expertise to provide coordinated care for people with complex needs. They work towards ensuring service users recover from mental health difficulties in a way that best meets their needs.
St. Pat’s Mental health uses the MDT in collaboration with the service user to develop a care plan. A careplan is a set of goals for the service user. The plan outlines the actions to be taken, along with the tools and supports that are used, to help the service user achieve their goals.
MDT members can include psychologists, psychiatrists, social workers, nurses, occupational therapists, pharmacists and more.
Every service user is also assigned a key worker, who is the health professional responsible for making sure that the service user and their MDT are working towards the same goals. The key workers oversees that the service user’s care plan is running effectively.
